The sample waiver/ release form is available for download. The form is intended to be used to meet the requirements of the RMS Swimming Insurance Program. It is necessary for all participants to sign, or have a legal guardian sign and keep on file with the school/ club administrator.

In the event of a claim, a signed waiver/ release form will be required by the insurance company to process the claim. Without a waiver/ release form a claim could be denied.

Instructions:

1. Open the Waiver/ Release Form (pdf) in Adobe Acrobat Reader. (Download Adobe Acrobat Reader)
2. Fill in the blanks on the waiver with the appropriate information.
3. Once the form has been customized to fit your program or association you may print it out and distribute to your participants for signatures.
4. Retain all copies of the form for each participant some place safe.
5. RMS recommends this process annually, at the beginning of your programs.
