**The following information is a brief description of the policy attributes. For more detailed policy information please contact a RMS customer service representative.**
TO QUALIFY FOR COVERAGE UNDER THE USSSA INSURANCE PROGRAM YOU MUST MEET THE FOLLOWING:
1. Your school must be a current member of USSSA.
2. You should have a signed waiver/release form on file for each participant.
3. All policy applicants must provide a copy of their completed waiver/ release form to RMS in order to complete the policy application process. Copies can be sent via email (info@theriskpeople.com), fax, or regular mail.

Descriptions of Coverage
- Coverage is provided for claims of negligence against the insured(s) by participants or any other person for Bodily Injury or Property Damage resulting from insured activities.
- Limited Coverage is included for Contractual Liability for claims arising from a written contract relating to insured activities.

- Coverage reimburses co-pays or deductibles not paid by primary health insurance provider.
