Please be sure to complete the application, have the company President or Chairman of the Board. (No other signature will be accepted!) Sign the application and circle the appropriate points adding up the total points which apply to your club.
In order to bind coverage, please complete the following:
- Download the application and fill it out completely.
- The application may only be signed by the President or Chairman of the Board. Incomplete applications will not be accepted.
- The original application MUST be returned by mail, with payment; no fax copies can be accepted.
- For new applications, payment must be made with a check or money order made payable to Risk Management Services, Inc.
- Both policies are Master Policies. Coverage will become effective the date we receive approval of the application and funds have been received. The coverage will continue for one year.
- Please contact RMS directly for overnight instructions.
